Consent to photograph or film
I, the parent or legal guardian of the camper named below, give Camp AllStar permission to photograph or film my camper during camp activities, and to use those photos or video on the Camp AllStar website, social media accounts, and printed promotional materials.
This permission is entirely optional. It is not a condition of enrollment, and I can withdraw it at any time by emailing campallstar3@gmail.com — future use will stop, though material already printed or shared before that request may not be recoverable.
I understand no payment is offered in exchange for this permission, and that Camp AllStar will not publish a photo of my camper without this release on file.
